At Active Lifestyle Physiotherapy we require your consent to collect personal information about you. Please read the following information carefully & sign below.

The main reason we collect information from you is to enable us to provide you with a quality service. The information we collect allows us to properly assess your situation, provide appropriate information & advice, & assist you in achieving your goals.

If, for any reason your claim is declined by insurance, you are responsible for the account.

We will also use the information you provide for administrative purposes in running our business & for correspondence with others involved in your health care including doctors, specialists, and other health professionals.

In order to provide our patients with the best possible care, Active Lifestyle operates on an appointment-based system. No-shows, late-comers and late cancellations impact greatly on our ability to provide a high quality service, by taking away an available spot for others, or by interrupting the smooth running of the practice.

Please provide us with at least 24 hours notice if you need to reschedule your appointment. Where possible, we will try to reschedule you to the next earliest convenient appointment, so as to avoid delays in your treatment and recovery.  

If less than 24 hours notice is given for a cancellation, a cancellation fee of $50.00 may be charged.  Consideration will be given for unavoidable circumstances. This fee is not covered by compensable bodies and must be paid by the patient. This fee also applies for no-shows.

If you are running late, please give us as much notice as possible. We will attempt to accommodate you, however, if there is a patient booked immediately after you, your appointment may understandably be cut short.  

People who repeatedly miss or reschedule appointments will regretfully be discharged from care as we realise you will not reach your health goals.
